Humana is seeking a Care Coordinator in Illinois to support the Illinois HealthChoice Medicaid Managed Care program. You will serve as an initial contact for new enrollees, perform HRAs, identify needs, and connect members with services and resources.

This is a field role with 75% local travel and requires coordinating care across providers and resources. You will work to establish a foundation for ongoing care, addressing barriers, and guiding members toward timely access to needed programs and

Qualifications

• Bachelor’s degree in social sciences, social work, human services, or related field.
• Unrestricted LPN license in Illinois with 1+ year experience in assessments and elder care.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsabilités

• Visit Medicaid enrollees in homes, facilities, and care settings (75% travel).
• Assess care needs to ensure least restrictive settings.
• Identify and resolve barriers to care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor progress against enrollee goals.
• Ensure progress through assessment, conversations, and active care planning.
