Job summary

Humana is seeking a Care Coordinator in Illinois to support the Medicaid HealthChoice program. You will engage with new enrollees, perform Health Risk Assessments, and connect members to needed services. This field role involves substantial travel and coordination with care teams to set up initial support.

The position emphasizes assessing needs, addressing barriers to care, and guiding families toward community resources to ensure a smooth transition into ongoing care management.

Qualifications

• Must reside in the state of Illinois.
• 1+ years of experience in health care and/or case management.
• 1+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.
• Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
• Experience using electronic medical records.

Responsibilities

• Visit Medicaid enrollees in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings – 75% local travel.
• Assess and evaluate enrollees’ needs via telephonic and face-to-face assessments to coordinate services.
• Plan and implement interventions to meet care needs.
• Coordinate services, monitor and evaluate the case management plan against enrollee goals.
• Assist enrollees with filing grievances and appeals.
• Provide information to enrollees related to MCO requirements, services, and benefits.

Education

Bachelor’s degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in Illinois with 1 year experience

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
