UnitedHealth Group is seeking a Field Care Coordinator (HIDE SNP) in Michigan to create person-centered, goal-oriented care plans and coordinate services for LTSS recipients. This field-based role emphasizes timely access to care and collaboration with the broader healthcare team.

Ideal candidates will manage care plans, advocate for patients, and work with social services to arrange needed supports. Relocation not mentioned; strong community health orientation is essential.

Qualifications

• Current, unrestricted independent licensure as an RN in Michigan, OR
• Master's degree and licensure as a Social Worker (LMSW/LCSW), OR
• Bachelor's degree and licensure as a Social Worker (LLBSW/LBSW).

Responsibilities

• Develop and implement care plan interventions as a single point of contact across the continuum of care.
• Communicate with stakeholders to ensure coordinated care and timely services.
• Advocate for persons and families to reflect their needs and choices in the care team.
• Assess, plan, and implement individualized care strategies aimed at the least restrictive care level.
• Identify barriers to care and provide appropriate care management interventions.
• Initiate referrals for social services, including financial and community support services.
• Provide resource support for local services based on assessments and plans.
• Manage the person-centered service plan throughout the continuum of care.
• Conduct home visits in coordination with the person and care team.
• Gather, document, and maintain member information to ensure compliance.
