Job summary

Right at Home Northwest Orange County, CA is seeking a Care Coordinator responsible for developing and managing client care plans while ensuring exceptional client experiences. The role involves consultations with clients and families, training caregivers, and managing office tasks.

The ideal candidate will have at least two years of caregiving experience and demonstrate strong communication and organizational skills. This position primarily works out of the local office with scheduled hours from Friday to Tuesday.

Qualifications

• Two years of caregiving experience or similar.
• Basic office and computer skills.
• Excellent interpersonal relations abilities.
• Valid driver’s license and insured automobile.
• Knowledge of common medical terminology.

Responsibilities

• Develop and manage the client care plan.
• Conduct training for new caregivers.
• Conduct client quality calls and follow up as needed.
• Maintain accurate client records.
• Educate clients on person-centered care.
