Job summary

UnitedHealthcare is hiring a Field Care Coordinator to assess, plan, and coordinate care for enrollees across Newport News, VA and surrounding areas. You will travel to homes, facilities, and community settings to conduct comprehensive assessments and develop person-centered care plans in collaboration with the Interdisciplinary Team.

You will advocate for members, support transitions, and promote self-management while following professional standards and policy guidelines.

Qualifications

• Bachelor's degree in Social Work or Human Services (or related field).
• 3+ years of experience in care coordination, behavioral health, and/or healthcare environment.
• 3+ years of experience with medically complex or elderly members.
• 1+ years of experience with MS Office (Word, Excel, Outlook).

Responsibilities

• Engage members face-to-face or by telecommunication to complete needs assessments.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants, and safety.
• Collaborate with internal care team, providers, and community resources to implement care plans.
• Provide education and coaching to promote member self-management.
• Support discharge planning and care transitions after ER/hospital/SNF events.
• Advocate for members and families to ensure needs and choices are supported.
