Job summary

A healthcare organization is seeking a Field Coordinator to assist in managing care for members. JOB SUMMARY:

The Field Coordinator is an essential team member of Centers Plan for Healthy Living’s Care Management department and assists with the care of all members enrolled in CPHL’s managed care plans. The Field Coordinator schedules and completes in-person visits as part of routine follow-up and collaborates with Care Management to ensure the member’s care needs are met.

PRIMARY RESPONSIBILITIES:

  • Schedule in-person visit appointments
  • Perform routine in-person visits at the member’s residence (home, nursing facility, adult care facility, etc.)
  • Review and complete in-person visit documentation with the member and/or their caregiver(s)
  • Maintain up-to-date and accurate documentation for assigned cases
  • Follow care coordination documentation guidelines, which includes, but are not limited to: documentation of problems, interventions and outcomes
  • Reach weekly caseload requirement and provide daily report of completed visits
  • Collaborate with Care Management to coordinate resources and services for members, including working with the member and caregiver(s) to ensure safety in the community, providing education, and obtaining relevant information to coordinate care
  • Participate in team meetings and on-going educational activities as required
  • Ensure that all job responsibilities are carried out in compliance with CPHL policies, NY State, CMS, and Federal regulations
  • Other assigned duties

Education And Experience:

Required: High School Diploma

Type of Experience:

  • Experience with customer service
  • Strong written and verbal communication skills
  • Organizational skills including attention to detail and multi-tasking skills

Preferred:

  • Experience working in managed care environment
  • Experience working with geriatric population
  • Bilingual

Specific Technical Skills:

  • Computer proficiency in Microsoft Office applications, Excel

  • None

Physical Requirements:

  • This position requires traveling to the member’s residence; must be able to travel between boroughs and have access to a vehicle or transportation
  • Must be able to operate a computer and telephone
