Companions & Homemakers HCA #2052 seeks a Field Coordinator to supervise in-home care, visit clients, and monitor caregiver performance. You will gather client needs and ensure matches align with company standards during field visits in CT.

You will document visits, escalate concerns, and participate in scheduling coordination while traveling within designated CT towns using your own vehicle. This role emphasizes hands-on field work and client-centered care.

Qualifications

• Ability to perform personal care tasks and client transfers as needed.
• Experience or comfort using lifting equipment like a Hoyer lift.
• Strong observational and time-management abilities.
• Willingness to travel within CT using own reliable vehicle.

Responsibilities

• Fill in for clients with last-minute openings and match care needs.
• Take on new clients until suitable caregiver match is found.
• Visit new clients within the first two weeks to validate info.
• Conduct home visits as requested by supervisors.
• Document relevant information from client visits or interactions.
• Escalate concerns to supervisor promptly regarding clients or staff.
• Follow weekly visits to monitor caregiver attendance and well-being.
• Prepare weekly activity reports and mileage when not driving a company vehicle.
• Develop knowledge of company policies and services and use training tools.
• Report any information affecting clients, caregivers, or the company.
