Job summary

UnitedHealthcare is seeking a Field Care Coordinator to support members in Berrien County, MI and surrounding areas. This field-based role combines home visits with office coordination to ensure compliant, person-centered care plans and effective service delivery.

You will work closely with care teams, manage plan implementations, and coordinate with social service agencies while traveling up to 75% to meet members where they are.

Qualifications

• Master’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
• Bachelor’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LLMSW, LBSW)
• 2+ years of experience in community health settings
• 1+ years of experience with local behavioral health providers and SDoH
• 1+ years of experience with long-term care and home-based services
• 1+ years of experience with MS Office
• Access to a designated quiet workspace at home and ability to secure PHI
• Ability to travel to Southfield, MI for quarterly meetings
• Must reside within the state of Michigan
• Access to reliable transportation and valid US driver’s license with travel up to 75%

Responsibilities

• Develop and implement care plans across the continuum of care as a single point of contact
• Communicate health-related information to stakeholders to ensure quality, coordinated care
• Advocate for patients and families to reflect needs and choices
• Assess, plan, and implement individualized care strategies
• Identify problems and provide care management interventions
• Identify and initiate referrals for social service programs and housing assistance
• Provide resource support for local services (CSHCS, employment, housing, etc.)
• Manage the person-centered service plan throughout care continuum
• Conduct home visits with the care team
• Conduct in-person visits (nursing homes, assisted living, hospital, home)
• Document and maintain all member information per state/federal guidelines
