Qualifications

• Bachelor’s Degree with at least 2 years of relevant experience
• RN licensed in Florida with relevant experience
• LPN licensed in Florida with relevant experience
• Case Worker with a minimum of 6 years of Health Services/Case Management Experience
• Prior experience with Medicare & Medicaid recipients
• UST reside in the listed areas within the region
• Microsoft Word, Excel and Outlook proficiency
• AHCA Fingerprint Required

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities; 75-90% local travel
• Assess and evaluate member’s needs to establish a member-specific care plan
• Ensure services are provided in the least restrictive setting to promote well-being
• Plan and implement interventions to meet identified needs
• Coordinate services and monitor the care plan against the member’s goals
• Guide members and families toward appropriate resources
• Interact with payer sources, providers and interdisciplinary teams as required by contract
