Job summary

Aspire Health Alliance in Marshfield, MA is seeking a Therapeutic Mentor for our home and community-based Therapeutic Mentoring (TM) program. The role provides one-to-one mentoring to youth to support goals in their behavioral health treatment plan and requires navigation of clinical documentation in AVATAR.

Responsibilities include traveling to youth homes, coordinating with schools and therapists, and maintaining charts.

Qualifications

• Bachelor's degree with 1 year experience working with children/adolescents/transition-age youth or
• Associate's degree with 1 year experience working with children/adolescents/transition-age youth or
• High school diploma or GED with minimum of two years’ experience working with children/adolescents/transition-age youth.

Responsibilities

• Travel to youth’s home and throughout the community
• Engage youth in home, school and/or community activities to teach new strategies and skills
• Support youth in recognizing individual skills and strengths
• Identify a natural support system and foster independence
• Coordinate with schools, therapists and care coordinators
• Maintain accurate clinical charts and timelines
• Participate in weekly supervision and staff meetings
• Meet agency productivity requirements
