Job summary

21st Century Home Health Services in Petaluma, CA is seeking a Speech Therapist to deliver in-home care across Solano County. You will assess and treat speech, language, cognitive and swallowing disorders, collaborating with an interdisciplinary team to maintain high-quality patient outcomes.

You must hold a CA Speech Therapist license, BLS/CPR, a valid CA driver's license, and be comfortable with flexible schedules, autonomy, and travel within the service area.

Qualifications

• Current unrestricted CA Speech Therapist License.
• BLS/CPR certification.
• Valid CA Driver's License and reliable transportation.
• Experience in home health care and high acuity patients preferred.

Responsibilities

• Provide skilled speech, language, cognitive and swallowing therapy in patients' homes.
• Collaborate with Case Manager and interdisciplinary care team to plan and adjust care.
• Educate patients and families, advocate for patient needs, and document progress.
