Job summary

21st Century Home Health Services (21HHS) is seeking a Speech Therapist/Language Pathologist to provide in-home therapy across the Bay Area. You will deliver patient-centered care, assess communication disorders, and design treatment plans tailored to individuals in their homes.

The role requires an unrestricted CA Speech Therapist License, BLS/CPR certification, and a valid CA Driver's License. You will work with an interdisciplinary team and have autonomy in scheduling within small coverage

Qualifications

• Current unrestricted CA Speech Therapist License required.
• BLS/CPR Certification required.
• Valid CA Driver's License required.

Responsibilities

• Visit patients in their homes as part of home health ST services.
• Collaborate with an interdisciplinary team to coordinate care.
• Adhere to state and federal homecare regulatory guidelines and company policies.
