Job summary

21st Century Home Health Services (21HHS) is seeking a Speech Therapist/Language Pathologist to provide in-home services across Solano, Napa, and Sonoma counties. You will evaluate and treat communication, cognitive, and swallowing disorders in a field-based setting, collaborating with an interdisciplinary team.

Requirements include CA licensure as a Speech Therapist, BLS/CPR, and a CA driver’s license. Prior home health experience is preferred; strong empathy, organizational skills, and patient

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and human behavior as it relates to illness/injury; cultural considerations in care.
• Experience in home health care is highly preferred.
• Experience with high acuity patients is a plus.

Responsibilities

• Provide skilled speech therapy services to assist patients with breathing, speaking, and swallowing.
• Evaluate and treat speech, language, cognitive and fluency disorders.
• Educate patients and families and advocate within the interdisciplinary care team.
• Collaborate with Case Manager to coordinate care and ensure quality outcomes.
