Job summary

21st Century Home Health Services in Marin County (San Rafael) is seeking a Speech Therapist/Language Pathologist for an in-person home health role. You will evaluate and treat communication disorders, collaborate with Case Managers, and support patients across a wide range of acuity in patients' homes.

Required CA Speech Therapist License, BLS/CPR, and CA Driver's License. The position offers autonomy, flexible scheduling, mileage reimbursement, and comprehensive benefits.

Qualifications

• CA Speech Therapist License required
• BLS/CPR Certification required
• Valid CA Driver's License required
• Experience in home health care preferred
• Experience with high acuity patients is a plus

Responsibilities

• Provide skilled speech-language pathology services in patients' homes in partnership with Case Manager
• Evaluate and treat speech, language, cognitive and fluency disorders
• Educate patients and families and act as a patient advocate within an interdisciplinary team
