21st Century Home Health Services in Vallejo, CA is seeking a Speech Therapist/Language Pathologist for a fully in-person home health role. You will visit patients in their homes within Solano County territory (Vallejo, American Canyon, Hercules) with occasional Napa County visits, delivering skilled speech and language interventions.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of home health regulatory guidelines preferred.

Responsibilities

• Provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
• Serve as educator, patient advocate and member of the interdisciplinary care team.
• Collaborate with Case Manager to ensure effective home visits and care plans.
