Qualifications

• HS diploma or equivalent.
• Minimum 3 years of experience with chronic illnesses/case management.
• Current, active RN license in applicable state(s).
• State-specific certification may be required by state law or contract.

Responsibilities

• Manage member cases within licensure scope and develop care plans.
• Perform telephonic or face-to-face assessments to coordinate and manage needs.
• Coordinate with non-RN clinicians and medical team to optimize care.
• Assign tasks to non-RN staff and monitor performance of care plans.
