Job summary

Elevance Health (Anthem) is seeking an LTSS Service Coordinator-RN Clinician to manage members' care across health, behavioral health, social services, and long-term supports. This field-based role travels to client sites and occasionally attends the office for meetings and training.

The RN will develop and monitor care plans, identify high-risk members, and ensure timely access to services while collaborating with physicians and care teams.

Qualifications

• Requires RN license in applicable state(s).
• Minimum of 3 years of experience working with individuals with chronic illnesses or disabilities in case management or similar roles.
• May require state-specific certification based on state law or contract.

Responsibilities

• Conduct face-to-face clinical assessments to identify and manage member needs across health care, behavioral health, social services, and long-term supports.
• Develop, monitor, and revise member care plans to optimize health outcomes.
• Identify high-risk members and coordinate care with the health team and member.
• Facilitate authorizations/referrals for services within benefits or contractual arrangements as appropriate.
• Supervise or coordinate non-RN clinicians involved in the member's care as needed.
• Collaborate with Medical Directors and interdisciplinary teams on care management plans.
