Activate Care in Massachusetts seeks a field-based Registered Nurse to perform in-home assessments for dually eligible members. You will document findings in the care management system and coordinate handoffs to the care team.

This role requires an active MA RN license, an Associate Degree in Nursing, and 1+ year home health experience. You will travel 70%+ across MA, with benefits including comprehensive medical, 401(k), PTO, and flexible daytime hours.

Qualifications

• Active Massachusetts RN license in good standing.
• Accredited nursing degree; BSN preferred.
• 1+ year home health experience desired.

Responsabilités

• Perform in-person health assessments at members' homes across MA.
• Document findings in GuidingCare and submit SOAP notes promptly.
• Maintain 48-hour documentation turnaround and audit readiness.
• Uphold safety, regulatory, and reporting standards.
