Activate Care is seeking a field-based Registered Nurse (RN) to travel across Massachusetts, performing in-person health assessments for dually eligible members. This role focuses on assessment and documentation, not ongoing case management.

Active MA RN license and 1+ year home health experience required; travel will be extensive, with a sign-on bonus of $2,000 upon hire and onboarding. Excellent health benefits and 401(k) offered.

Competenze

• Active Massachusetts RN license in good standing.
• Associate Degree in Nursing required; BSN preferred.
• Valid driver’s license, reliable vehicle, and active auto insurance.
• 1+ years of home health clinical experience required.

Mansioni

• Conduct in-person assessments in members’ homes and community settings across MA.
• Document assessment findings in the health plan’s care management platform and send SOAP notes.
• Submit complete documentation within 48 hours of each assessment and address audit findings promptly.
• Uphold safety, HIPAA, NCQA standards and mandatory training requirements.
• Travel 70%+ to members’ homes and maintain independent field schedule.
