Activate Care is seeking a field-based Registered Nurse (RN) to travel across Massachusetts, performing in-person health assessments for dually eligible members. You will document findings in GuidingCare and hand off to care management, while complying with HIPAA and safety standards.

A Massachusetts RN license and access to a reliable vehicle are required. This role emphasizes accurate assessment and timely documentation, with onboarding focused on Full-Time positions and a $2,000 sign-on bonus

Qualifications

• Active Massachusetts RN license (or higher) in good standing.
• Accredited Associate Degree in Nursing completed; BSN preferred.
• Valid driver’s license and reliable vehicle required.

Responsabilités

• Travel to members’ homes across Massachusetts to perform in-person health assessments.
• Document assessment findings accurately in the health plan platform (GuidingCare) and send SOAP notes.
• Submit complete documentation within 48 hours and address any audit findings promptly.
• Follow safety guidelines, HIPAA, and regulatory requirements during field visits.
