Activate Care is seeking a field-based Registered Nurse to travel across Massachusetts, conducting in-person health assessments for dually eligible members. This role focuses on timely assessment, precise documentation, and smooth handoffs to the care-management team.

Active MA RN license and an Associate Degree in Nursing are required, with 1+ years in home health. Travel, HIPAA compliance, and straight-ahead field work are core aspects of this position.

Qualifications

• Active Massachusetts RN license in good standing.
• Associate Degree in Nursing required; BSN preferred.
• 1+ years of home health nursing experience required.

Responsabilités

• Conduct in-person home assessments for target members.
• Document findings in GuidingCare and send SOAP notes.
• Submit documentation within 48 hours; meet quality standards.
• Ensure safety, mandated reporting, and HIPAA compliance.
• Coordinate with care management team for handoffs.
