Job summary

Elevance Health is seeking a LTSS Service Coordinator – RN Clinician (RN Case Manager) to provide comprehensive case management across physical health, behavioral health, and long-term supports in Western Ohio. The role involves field-based visits, care plan development, and coordination with healthcare teams to optimize member outcomes.

The ideal candidate will hold a valid RN license in Ohio, have 3+ years in case management or related roles, and preferably a BA/BS in Health or Nursing.

Qualifications

• Requires a high school diploma or GED and minimum 3 years of experience in working with individuals with chronic illnesses or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active, valid RN license in Ohio state(s) required.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify, evaluate, coordinate and manage members' needs across physical health, behavioral health, social services and long-term supports.
• Manage members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Obtain thorough member history to develop individual care plans.
• Establish short and long-term goals with the member and care team.
• Develop and oversee care plans and ensure access to services.
• Assist with implementation of member care plans by facilitating authorizations/referrals as appropriate.
• Collaborate with Medical Directors and Inter-Disciplinary Teams on care management.
