Job summary

Elevance Health is seeking a Field-based LTSS Svc Coordinator- RN Clinician to manage members' care plans and coordinate services across health, behavioral, social, and long-term supports. The role involves telephonic and in-person assessments, collaboration with clinicians, and oversight of non-RN staff involved in care.

The RN will develop individualized care plans, set goals with members and families, and ensure access to services within benefits.

Qualifications

• Requires a HS diploma or equivalent and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in applicable state(s) required.
• MA/MS in Health/Nursing preferred; state-specific certification may be required by law/contract.

Responsibilities

• Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member needs.
• Identify high-risk members and coordinate care with health care team.
• Manage members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Obtain thorough member history to develop individualized care plans.
• Establish short and long-term goals with the member and care team.
