UnitedHealth Group is seeking a Field-Based RN Case Manager in Fort Worth, TX. The role focuses on education, advocacy, and connecting members with resources to ensure safe transitions from hospital to home. You will visit members in the hospital before discharge and coordinate care with families and care teams.

This full-time position requires an unrestricted Texas RN license and 3+ years of clinical experience in relevant settings. Travel within a 50-mile radius is expected, with day shifts.

Formación

• Current, unrestricted RN license in the State of Texas.
• 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care, or case management.

Responsabilidades

• Visit members in the hospital prior to discharge.
• Provide a continuum of quality care through close communication with members via in-person or phone interaction.
• Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.
• Assist members with the transition from a care facility back to their home.
