Ethos is seeking an experienced RN Home Health Case Manager for a field-based role serving Fort Worth, TX. The ideal candidate will manage a patient caseload, perform skilled nursing visits, and develop individualized plans of care in collaboration with the care team.

Active RN license and recent home health case management experience are required. Strong assessment skills, documentation habits, and independent field work are essential for success.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, recertifications, and discharges.
• Develop and coordinate individualized plans of care with physicians, patients and families.
• Complete accurate clinical documentation including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Educate patients and caregivers on disease management, medications, safety, and goals.
• Coordinate care with therapy, social work, aides, physicians and leadership.
• Maintain compliance with home health regulations and standards.
