Qualifications

• Current, unrestricted RN license in the State of Texas.
• 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management.
• Proven ability to communicate complex information clearly and to interpret complex information from others.
• Reliable transportation to travel within a 50 mile radius of home.

Responsibilities

• Visit members in the hospital prior to discharge.
• Provide a complete continuum of quality care through close communication with members.
• Support members with condition education, medication reviews and connections to resources.
• Assist members with transition from a care facility back to home.
