Job summary

Sora Resource Group is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. You will perform skilled nursing visits, assessments, and plan-of-care coordination in the patient’s home, collaborating with physicians, families, and the care team.

The ideal candidate holds an active Texas RN license, has recent Home Health Case Management experience, and can manage a caseload independently while ensuring accurate

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong Medicare home health, OASIS, and plan-of-care knowledge.
• Ability to work independently with strong team communication.
• Valid driver’s license and ability to travel locally.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Provide patient and caregiver education on disease management, medications, safety, and goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory and quality standards.
