Job summary

UnitedHealthcare is seeking a Field Based Registered Nurse to support members in the Fort Worth, TX area. This role requires traveling to member homes and community sites, with a home-based office setup and up to 75% travel within the region.

You will assess care plans, coordinate referrals to social services, advocate for patients, and maintain clear communication with all stakeholders to ensure timely, quality care.

Qualifications

• Current unrestricted Texas RN license.
• 2+ years of experience in a community health or healthcare setting.
• Valid driver’s license and reliable transportation; ability to travel up to 75% within the assigned region.

Responsibilities

• Assess, plan and implement care strategies individualized by patients and directed toward appropriate levels of care.
• Identify and initiate referrals for social service programs (financial, psychosocial, community, state).
• Manage the care plan across the continuum as a single point of contact and communicate required health information to ensure coordinated care.
• Advocate for patients and families to ensure needs and choices are represented by the health care team.
• Travel 50-75% within the Tarrant County, TX area and surroundings.
