Job summary

UnitedHealthcare is seeking a Field-Based Registered Nurse for the Fort Worth, Texas area. This role offers home-based flexibility with telecommuting options within commutable distance to Tarrant County.

You will travel up to 75% within the region to visit Medicaid members in their homes and community settings. Responsibilities include assessing and planning individualized care, coordinating referrals to social services, and advocating for patient needs while ensuring quality, coordinated care.

Qualifications

• Current unrestricted Texas RN license is required.
• 2+ years of experience in a community health or healthcare setting.
• Proficiency with Microsoft Office applications.

Responsibilities

• Assess, plan and implement care strategies tailored to patients and the least restrictive level of care.
• Identify and initiate referrals for social service programs including financial and psychosocial support.
• Manage the care plan across the continuum and coordinate communication among stakeholders.
• Advocate for patients and families to ensure needs are represented by the health care team.
• Travel 50-75% within the Tarrant County, TX area and surrounding areas.
