Job summary

UnitedHealth Group is seeking a Field-based Registered Nurse in Seattle, WA to visit members at home, assess health status, identify gaps in care, and educate patients for self-management. You will coordinate with care teams and make referrals to Home Health Aides or Meals on Wheels as needed.

The role requires an active WA RN license, BLS or ability to obtain, 1+ year of clinical, home-care experience, and a valid driver’s license with reliable transportation. Schedule is Mon–Fri, 8:00 a.m.

Qualifications

• Current, unrestricted RN license in WA.
• Basic Life Support certification or ability to obtain by start date.
• 1+ years of clinical experience.
• Experience in home care/home visits.
• Reside in or near Seattle, WA.
• Driver's license and reliable transportation.

Responsibilities

• Visit members in their homes to assess health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education for self-management.
• Make referrals to outside sources.
• Ensure complete continuum of quality care via in-person or phone contact.
• Support members with education, medication reviews and connections to resources.
