Qualifications

• Must hold an Indiana RN license without restrictions.
• At least 2 years of clinical experience in case management or similar care services.
• Proficient in Word, Excel, and Outlook.
• Strong communication and interpersonal abilities.

Responsibilities

• Assess and resolve barriers to care to support member wellness.
• Coordinate medical and non-medical services for members.
• Collaborate with ICT and care team to address identified needs.
• Coordinate with Medicare payers and providers for eligible members.
• Serve as primary contact for ICT and coordinate with members and external resources.
