Job summary

FreedomCare is seeking a Care Manager, RN for the Connecticut team. This field-based role conducts in-home assessments, orientations for new patients and caregivers, and ongoing care management to ensure safe, person-centered care.

You will travel regularly to patient homes in New Haven and surrounding areas, coordinate with interdisciplinary teams, develop care plans, and ensure regulatory compliance while coaching families and documenting care.

Qualifications

• Diploma from an accredited nursing program.
• New graduates welcome!
• Active RN license in the State of Connecticut (CT) is required or in process.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation, ensuring forms and procedures are followed.
• Develops and manages patient's plan of care.
• Makes revisions and documents changes as needed.
• Coordinates care with physicians and other disciplines involved in the client’s care.
• Provides ongoing coaching to drive best practices and connect families to resources.
• Reviews caregiver notes daily and addresses changes in condition or environment.
• Ensures compliance with regulatory standards and company policies.
• Performs other job-related duties as assigned by supervisor.
