Job summary

FreedomCare is seeking a Nurse Case Manager to conduct in-home clinical assessments and develop care plans for patients in Connecticut. You will coordinate with physicians, caregivers, and internal teams to ensure safe, person-centered care and smooth service initiation.

Ideal candidates are graduates of accredited nursing programs with CT RN licensure, strong assessment and documentation skills, and ability to travel across West Hartford, New Haven, Bridgeport, Waterbury and New Britain areas.

Qualifications

• Graduate of an accredited nursing program.
• Active and unrestricted RN license in Connecticut.
• Strong clinical assessment, communication, and documentation skills.
• Willingness to travel within assigned geographic area.

Responsibilities

• Conduct in-home clinical assessments and help patients and caregivers begin services.
• Develop, implement, and update individualized plans of care.
• Build ongoing relationships with patients and caregivers through home visits and follow-up.
• Use clinical judgment to identify changes in condition, safety concerns, and barriers to care.
• Coordinate care with physicians, caregivers, internal teams, and community resources.
• Educate and coach patients and caregivers on care plans and resources.
• Review caregiver documentation and address clinical changes or concerns.
• Maintain accurate clinical documentation and ensure compliance with regulations and standards.
• Collaborate with interdisciplinary team to improve patient outcomes.
