Job summary

FreedomCare LLC is seeking a Care Manager, RN to support Massachusetts members with in-home assessments and care coordination. This field-based role requires regular travel across assigned MA territories to activate services, educate families, and monitor plans of care.

You will collaborate with interdisciplinary teams to ensure safe, effective, person-centered care, and maintain thorough clinical documentation and timely follow-ups. Massachusetts RN license and mobility are essential.

Qualifications

• Current, unrestricted RN license in Massachusetts.
• At least two years of direct care to elders or adults with disabilities preferred.
• TB screening completed before assignment and ongoing per CDC/MassHealth rules.
• CORI, SORI, OIG checks and references completed.
• Must have reliable transportation and be able to travel to member settings.

Responsibilities

• Conduct initial in-home clinical assessments and coordinate service activation.
• Develop, implement, and adjust individualized care plans as needs change.
• Educate patients and caregivers on care needs, safety, and service requirements.
• Coordinate care with physicians, teams, and community resources.
• Provide ongoing education to support caregiver competency and best practices.
• Review caregiver documentation and respond to changes in patient condition.
• Identify urgent issues and escalate per clinical protocols.
• Document assessments, visits, care-plan updates, and follow-up actions timely.
