Job summary

FreedomCare is seeking a Care Manager, RN for the Massachusetts team. This field-based role requires regular travel throughout an assigned Massachusetts territory to conduct in-home visits and support member care, covering areas such as North Shore, Boston Proper, South Shore & Cape Area, Worcester, and Western Massachusetts.

The RN will perform initial in-home assessments, orientations for new patients and caregivers, and ongoing care management, coordinating with physicians and care teams

Qualifications

• Current, unrestricted Massachusetts RN license.
• 2+ years of direct care experience for elders or adults with disabilities preferred.
• TB screening completed before assignment and ongoing screening per CDC/DPH/MassHealth requirements.
• CORI/SORI/OIG checks, references, and employment history verified.
• Must have valid driver’s license, reliable transportation, and ability to travel within MA.

Responsibilities

• Conduct initial in-home clinical assessments and coordinate service activation.
• Develop and manage individualized plans of care with revisions as needed.
• Educate patients and caregivers on care needs, safety, and service requirements.
• Coordinate care with physicians, clinical teams, and community resources.
• Provide ongoing education to support caregiver competency and patient outcomes.
• Review caregiver documentation and respond to changes in patient condition.
• Escalate urgent issues per clinical protocols and ensure timely interventions.
• Document assessments, visits, and follow-ups accurately within timeframes.
• Assist with authorization, renewals, and continued-service processes.
• Maintain compliance with regulatory requirements and company policies.
• Perform other duties as assigned by the supervisor.
