Job summary

FreedomCare is seeking a Care Manager, RN to perform in-home assessments and coordinate care for members across a MA field territory. This field-based role requires regular travel to patient homes, conducting initial assessments, teaching caregivers, and guiding care plans within the MassHealth AFC framework.

The RN will collaborate with patients, families, and interdisciplinary teams to ensure safe, person-centered care and timely service delivery. MA licensure and driving ability are essential.

Qualifications

• Current, unrestricted license as a registered nurse (MA).
• At least two years of direct care experience for elders or adults with disabilities preferred.
• TB screening completed before assignment and ongoing screening as per CDC/MA guidelines.
• CORI, SORI, OIG checks and reference/employment history checks required.
• Must have valid driver’s license, reliable transportation, and ability to travel across MA.

Responsibilities

• Perform initial in-home clinical assessments and activate services.
• Develop and manage individualized care plans; document changes as needs evolve.
• Educate patients, caregivers, and internal teams on care requirements and safety.
