Job summary

FreedomCare in Massachusetts is seeking a Care Manager, RN to conduct in-home assessments, orient new patients and caregivers, and provide ongoing care management.

You will collaborate with patients, families, and interdisciplinary teams, develop plans of care, monitor health status, and ensure safe, person-centered services.

This field-based role requires travel across assigned MA territories; MA licensed RN with related experience preferred; competitive pay and benefits.

Qualifications

• Current, unrestricted RN license issued by MA Board of Registration in Nursing.
• Two years of recent direct care experience with elders or adults with disabilities preferred.
• Completion of tuberculosis screening before assignment and ongoing screening per CDC/Massachusetts requirements.
• CORI, SORI, OIG checks and reference/employment-history/licensure checks.
• Must have valid driver’s license and reliable transportation.

Responsibilities

• Conduct initial in-home assessments and coordinate service activation with proper documentation.
• Develop, implement and manage individualized plans of care; revise as needs change.
• Educate patients, caregivers, and internal teams on care needs and safety requirements.
• Coordinate patient care with physicians and community resources through timely communication.
• Provide education and coaching to promote caregiver competency and use of best practices.
• Review caregiver documentation and respond to changes in patient condition.
