Job summary

FreedomCare is seeking a Care Manager for Connecticut to assess home care needs, orient new patients, and train caregivers to ensure high-quality service for clients in their homes. The role covers multiple counties within Connecticut and requires travel throughout the state with mileage reimbursement.

Ideal candidates have 2+ years of experience with frail or elderly adults, strong communication skills, and a valid driver’s license.

Qualifications

• 2+ years of experience working with frail or elderly disabled adults
• Field experience in a medical or case management environment preferred
• Experience with data input and case note entry; Salesforce experience is a plus

Responsibilities

• Evaluate the home care needs of the client
• Assess the environment
• Train caregivers to provide care
• Initial assessments during orientations
• Travel within the state for field visits
