Qualifications

• Valid and active Registered Nurse (RN) license in the State of Connecticut.
• Ability to articulate cases through effective case summaries.
• Ability and flexibility to travel within an assigned geographic area.

Responsibilities

• Conduct initial in-person clinical assessments and coordinate service activation.
• Develop, implement, manage, and revise the patient’s plan of care.
• Provide ongoing coaching to promote best practices and encourage education.
