Humana is seeking a Field Care Manager Nurse 2 to assess member needs and guide families toward appropriate care resources, in home or community settings and by phone. The role requires coordinating with the care team to develop care plans and ensure optimal outcomes.

The position emphasizes managing complex cases, advocating for access to services, and ensuring care is delivered in the least restrictive setting. A Michigan RN/APRN license and 2+ years in health care/case management are required.

Qualifications

• Must reside in Michigan (Wayne or Macomb Counties).
• Active Michigan RN or APRN license with no disciplinary action.
• 2+ years’ experience in Health Care and/or Case Management.
• Ability to travel to homes and community settings for face-to-face assessments.
• Experience with adult population and disease management.
• Knowledge of community health and social service agencies and resources.
• Ability to use electronic information systems including EMR.
• Proficient keyboard and web navigation; MS Word, Outlook, Excel.

Responsabilités

• The RN Field Care Manager will manage a caseload and complete assessments with members in home or community settings, and via telephone.
• Provides clinical support and guidance for members with medical complexity; helps develop and coordinate care plans; ensures appropriate services are provided and barriers to care are addressed.
• Reviews, assesses, and completes medical complexity attestations and clinical oversight activities.
• Ensures services are delivered in the least restrictive setting to maintain or improve well-being.
• Develops and modifies the Individual Care Plan with the care team (informal caregiver, coach, PCP, etc.).
• Supports members and/or caregivers using an interdisciplinary approach to access social, housing, educational and other services irrespective of funding source.
• Serves as primary contact for the Interdisciplinary Care Team (ICT) coordinating with member, ICT participants and external resources.
