Humana At Home is seeking a Field Care Manager Nurse 2 (temporary contractor) to conduct in-home assessments for Medicare/Medicaid members, with travel to Wayne or Macomb counties in Michigan. The role requires a Registered Nurse with 2+ years in health care or case management.

Based from a home office, you will develop care plans, coordinate with an interdisciplinary team, monitor patient needs, and advocate for services to improve wellness.

Qualifications

• Must be a Registered Nurse with 2+ years in health care or case management.
• Active Michigan RN license with no disciplinary action.
• Must reside in Michigan with ability to travel to Wayne or Macomb Counties for home visits.
• Experience with adult population and disease management.
• Knowledge of community health resources and agencies.
• Excellent communication and interpersonal skills with ability to build rapport.
• Ability to work with minimal supervision and use electronic medical records.
• Proficient with EMR and MS Office products.
• Full-time (40 hours/week) Mon–Fri.
• Driver's license, vehicle insurance, and access to a personal vehicle.
• Home office setup with reliable high-speed internet; TB screening may be required.

Responsabilités

• Manage a caseload and complete assessments in home or community settings and by phone.
• Provide clinical support and guide care plans to ensure appropriate services.
• Address barriers to care and advocate for optimal outcomes.
• Review medical complexity attestations and clinical oversights.
• Ensure services are delivered in the least restrictive setting to promote well-being.
• Develop and modify Individual Care Plans with the care team.
• Coordinate with the Interdisciplinary Care Team and outside resources to meet member needs.
