Humana Healthy Horizons in Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) to assess and guide members toward appropriate health and community resources, supporting Indiana PathWays for Aging. You will work in a hybrid model, combining remote work with on-site member visits.

Qualifications

• Licensed RN in Indiana (no restrictions).
• 2+ years clinical nursing with case management or similar health care services.
• Strong communication and interpersonal skills, ability to build rapport with diverse stakeholders.
• Ability to manage multiple priorities in a fast-paced environment.
• Proficiency with electronic medical records and MS Office suite.
• Residence in Indiana.

Responsibilities

• Coordinate and manage member care across medical and non-medical services.
• Develop longitudinal, trusting relationships with members to improve care quality and continuity.
• Collaborate with ICT and other care team staff to address identified needs.
• Coordinate with Medicare payers and providers for eligible members.
• Serve as primary point of contact for the ICT and coordinate with members and resources.
