Qualifications

• Current, unrestricted RN license in Texas
• 3+ years clinical experience in hospital/acute care/home health or case management
• Ability to communicate complex information clearly and interpret complex information from others
• Flexible with daily schedule and able to adapt to changing priorities
• Proficiency with electronic medical records, email, internet research, and online calendars

Responsibilities

• Visit members in hospital prior to discharge
• Provide complete continuum of quality care through close communication with members in person or by phone
• Support members with education, medication reviews and connections to resources (e.g., Home Health Aides, Meals on Wheels)
• Assist members with transitioning from care facilities back to their homes
