UnitedHealth Group is seeking a Registered Nurse in the Mission, TX area with experience in community health to coordinate care and connect members to resources. Remote work flexibility is offered within commuting distance.

The role requires an active Texas RN license, general healthcare experience, and ability to travel up to 75% within the Mission area. A strong knowledge of HCBS and Medicaid-related services is preferred.

Qualificações

• Current unrestricted Texas RN license.
• 2+ years of experience in a community health or healthcare setting.
• Intermediate level of proficiency with Microsoft Office applications.
• High-speed internet at residence.
• Access to reliable transportation and valid US driver's license with ability to travel within assigned territory.

Responsabilidades

• Assess, plan, and implement person-centered strategies.
• Connect members with financial, psychosocial, community, and state resources.
• Coordinate care across the continuum and serve as a key point of contact.
• Advocate for members and families to ensure their needs and preferences are heard and supported.
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
• Travel up to 75% of the time within the Mission, TX area and surrounding areas.
