Job summary

UnitedHealthcare is hiring in the Greater Houston area for a community health nurse role. You will assess and plan person-centered strategies, connect members to resources, and coordinate care across the continuum.

Expect travel within Houston and surrounding areas as a key part of the job. The position requires an unrestricted Texas RN license and at least 2 years in a healthcare setting, with proficiency in MS Office and reliable transportation.

Qualifications

Current unrestricted Texas RN license.
2+ years of experience in a community health or healthcare setting.
Intermediate level of proficiency with Microsoft Office applications.
Reliable transportation, a valid driver's license, and ability to travel within assigned territory.
High-speed internet at residence.

Responsibilities

Assess, plan, and implement person-centered strategies.
Connect members with financial, psychosocial, community, and state resources.
Coordinate care across the continuum and serve as a key point of contact.
Advocate for members and families to ensure their needs and preferences are heard and supported.
Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
Travel 50% of the time within the Houston area and surrounding areas.
