This full-time, day shift role requires 2+ years in community health, an unrestricted Texas RN license, and a valid driver’s license with reliable transportation; travel will be 50-75% within Tarrant County and surrounding areas.

Formación

• Current unrestricted Texas RN license is required.
• Minimum 2+ years of experience in community health or healthcare setting.
• Valid driver's license and access to reliable transportation are required.

Responsabilidades

• Assess, plan and implement care strategies individualized for patients.
• Identify and initiate referrals for social service programs.
• Manage care plans throughout the continuum as a single point of contact.
• Communicate with stakeholders to ensure coordinated care.
• Advocate for patients and families to meet their needs.
