Job summary

UnitedHealthcare is seeking an RN Clinical Coordinator for field-based care across Camden and surrounding counties, NJ. You will manage a panel of members with complex medical and behavioral needs, focusing on coordination of medical, behavioral, and social services to improve quality and appropriate utilization.

Expect ~80% field visits to members in homes or long-term care facilities. Requires NJ RN license, 2+ years clinical experience, travel up to 80%, and MS Office proficiency.

Qualifications

• Current, unrestricted RN license in New Jersey is required.
• 2+ years of clinical experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Travel up to 80% within assigned territory to meet members/providers.

Responsibilities

• Conduct comprehensive needs assessments for a panel of members.
• Develop person-centered care plans addressing medical, behavioral, and social needs.
• Collaborate with care team, providers, and community resources to implement plans.
• Educate and coach members on self-management and lifestyle changes.
• Coordinate care transitions and discharge planning after ER/inpatient/SNF events.
• Advocate for member needs within the health care team.
