Activate Care in Massachusetts seeks a registered nurse to travel to members' homes across the state for in-home health assessments under a managed care contract. The role emphasizes timely assessment, precise documentation, and seamless handoff to care management teams.

The RN must hold a Massachusetts license in good standing and have at least 1 year of home health experience; BSN is preferred. Flexibility and strong communication skills are essential for success.

Competenze

• Active MA RN license in good standing
• 1 year home health clinical experience preferred
• Experience with dual-eligible populations (Medicaid/Medicare) preferred

Mansioni

• Travel to members' homes across Massachusetts to perform health assessments
• Document assessments in GuidingCare or equivalent health plan platform
• Submit SOAP notes to Care Manager within required timelines
• Maintain high quality and complete documentation; adhere to HIPAA and NCQA standards
• Routinely schedule visits, manage multiple assessments, and escalate safety concerns as needed
• Maintain licensure and stay current with required trainings
