Job summary

Activate Care is seeking a field-based Registered Nurse (RN) to travel across Massachusetts and perform in-person health assessments for a managed care health plan’s dually eligible members. The role emphasizes accurate assessments, documentation, and timely hand-off to care management, with no ongoing care planning involved.

The RN will operate independently in members’ homes, manage a flexible field schedule, and document in GuidingCare or similar platforms while maintaining high-quality

Qualifications

• Active Massachusetts RN license in good standing required.
• Associate Degree in Nursing required; BSN preferred.
• Experience with dual-eligible, Medicaid, or Medicare populations preferred.
• Proficient with electronic documentation systems and secure virtual desktop environments.

Responsibilities

• Conduct In-Person Assessments in members' homes and community settings.
• Document assessment findings in care management platform and send SOAP notes.
• Submit complete documentation within 48 hours of each assessment.
• Uphold safety and mandated reporting standards and HIPAA compliance.
• Ensure regulatory alignment and complete mandatory trainings.
