Job summary

Public Health Management Corporation (PHMC) is seeking a full-time Reentry Specialist for the Delaware County Reentry Project (DCRP). This in-person role bridges incarceration and community reintegration, working with probation, case management, peer supports, and community providers to address housing, employment, behavioral health, and supervision compliance.

You will conduct outreach, coordinate services, and provide continuity of care across jail, home, and community settings, including

Qualifications

• Two (2) years of experience with justice-involved individuals and/or substance use or mental health disorders.
• Must be able to work a flexible schedule.
• Familiarity with probation/parole systems is strongly preferred.
• Experience in reentry, case coordination, peer support, or community-based social services is preferred.

Responsibilities

• Complete weekly face-to-face meetings and daily phone contacts with participants.
• Identify community linkages to support recovery, employment, housing, and education.
• Engage family members to address Social Determinants of Health barriers.
• Provide services in jails, homes, treatment facilities, and community sites.
• Escort participants to appointments for additional services.
• Document participant services in PHMC records.
• Provide after-hours on-call support on a rotating basis.
